I used to work at a children's activity center that had indoor caving made out of fiberglass .
I remember a staff member becoming stuck and had to be cut out.
I also had to deal with nose bleeds, vomit and panic attacks .

So far, it sounds odd. Crush injury/rhabdomyolysis is possible. I'm not going to criticise anyone, nor am I going to detail things that might have been done differently. It seems to have been an extremely unusual incident, beyond the usual competence of 'normal' first responders in the UK.
When the coroner has blessed everyone with hindsight, people may wish to criticise. I think that would be wrong. There are probably lessons to learn but these should be lessons learnt by emergency response systems as a whole, not punitive 'lessons' imposed on individuals.
