Yesterday I walked up to Gaping Gill and shouted at a guy throwing rocks down the hole, explaining to him that there could be people below in the chamber.

Well as you mention it, it could be argued that the artificial cave used at Hidden Earth for people to crawl through is an "overground/above ground" cave.
