Thinking about the whole system is good, because it gets you thinking about failure modes

The common ones appear to be human:

Error of judgement,
Lack of concentration,
Fatigue,
Inadequate understanding,
Lack of training,
Lack of experience,
Stupidity
etc.

Another one would be "ingnorance" i.e. inability to know (or grasp)  the consequences, probably linked into No.5, "training".
