Didn’t a well known dales caver once get stuck in a fake cave and have to be extracted?! Name deliberately omitted 😉. If it’s meant for kids, it’s meant for kids!

That is most sombre, I apologise for the chuckle I have when I read indoor caving rescue thinking it was a simple big guy underestimating the space.

Whatever, it’s a tragic incident that has devastated the family and the company to whom the venue belonged. Thoughts and condolences to all involved.

I know I shouldn't pontificate on these things, but sounds like at toxic shock syndrome type thing to me, resulting from some sort of suspension trauma / crush injury (i.e. being stuck in very awkward position for a long time).

So far, it sounds odd. Crush injury/rhabdomyolysis is possible. I'm not going to criticise anyone, nor am I going to detail things that might have been done differently. It seems to have been an extremely unusual incident, beyond the usual competence of 'normal' first responders in the UK.
When the coroner has blessed everyone with hindsight, people may wish to criticise. I think that would be wrong. There are probably lessons to learn but these should be lessons learnt by emergency response systems as a whole, not punitive 'lessons' imposed on individuals.

From news reports, he seems to have slipped & ended up with his legs folded up in front of him (foetal position), which would explain why it took so long to get him out & could explain the toxic shock afterwards
