I've gone in for a ride on this more than once - me and Domee did the whole circuit a few weeks ago. At the top and bottom the damn thing goes sideways round huge pulley wheels. Screw it up and you get chopped in half, though there is an emergency brake

Yeah. That would my worry. Getting caught in the gubbins rather than plumetting down teh shaft.
The lift on BBC link isn't a man engine though. They were originally designed in Germany before being fitted here (also has details of the Levant disaster):

The paternoster lift was first built here, but more are found in Germany nowadays:
